REGISTRATION FORM
- Homework assistance is provided for the students
- Students are brought to and from After School classes
- After Care students are given priority on Breaktime Days
- Sign up for any or everyday of the week
- Hours until 6:00pm Mon. – Thurs.
- Hours until 5:00pm Fri. (3:00pm during early Fridays)
- AS NEEDED BASIS: PAYMENT DUE AT PICK UP!! Please see below for AS NEEDED rates.
- Fall Semester: Tues. 9/1/11 – Fri. 1/20/12
- Grades N (age 4) and up
- REGISTER TODAY WITH A $175 DEPOSIT

After Care As Need Basis
RATES For Grades N – 3:
- $ 7.00 for pick up by 4:00 pm
- $ 8.50 for pick up by 4:15 pm
- $ 9.50 for pick up BY 4:30 pm
- $16.00 for pick up BY 5:00 pm
- $19.00 for pick up BY 5:30 pm
- $24.00 for any pick up after 5:30 pm
- PAYMENT DUE AT PICK UP!!
- All students must be picked up no later than 6:00pm
RATES For Grades 4 and up:
- $ 9.00 for pick up BY 4:30 pm
- $12.00 for pick up BY 5:00 pm
- $16.00 for pick up By 5:30 pm
- $21.00 for pick up after 5:30pm
- PAYMENT DUE AT PICK UP!!
